Insurance Policy
It is your responsibility to provide us with accurate and complete information concerning your primary and secondary insurance medical benefits, including referral documents from other providers. Current photo ID and insurance cards should be presented at each office visit. As a courtesy, we will file your insurance claim for you. Please note, Hale Men’s Clinic does not take Medicare or Medicaid Insurance.
Financial Agreement
The patient is responsible for the payment of services rendered if the services are not covered by insurance for any reason. Hale Men’s Clinic, LLC, and KRISTIAN FERRY, MD is a participating provider with most health plans, however, participation is subject to change. It will be the patient's responsibility to verify with the insurance carrier the plan participation status of KRISTIAN FERRY, MD prior to a service being rendered. Insurance will be according to the billing/payment guidelines of the primary insurance contract. If a referral is required by your plan it is the patient's responsibility to obtain or insurance may not pay my claims.
